• File a complaint if you believe your privacy rights have been violated.

Your Choices

For certain health information, you can tell us your preferences about how we share your information.

This includes:

• Sharing information with family members, friends, or others involved in your care.
• Sharing information in disaster relief situations.
• Fundraising communications, where you may request not to be contacted.

If you are unable to communicate your preferences, we may share information if it is in your best interest or when necessary to prevent a serious threat to health or safety.

Students

If you prefer that a student not observe or participate in your care, please notify your provider.

Unless you choose to opt out, AI tools may be used as part of your dental services in accordance with applicable privacy requirements.

Our Responsibilities
